Vocational Training Schemes for Practice Doctors

Appraisals and Log Books

All doctors now need annual appraisals, which will start from the ST3 year on an annual basis. During the ST 1 and 2 years, during each specialty posting, you will be appraised by your educational supervisor, with some input from your GP trainer with whom you will be posted for your ST3 year.

It is important to start keeping an educational log of all your learning, as well as skills gained during your postings. The SHO attribute guide was created for this purpose and covers all the skills and knowledge you need to acquire during your 6 month post. This can be downloaded from the SHO attribute guide link at the North West Deanery website using this link.

During each 6 month post, you are required to formally meet with your educational supervisor, once at commencement, second time, half way through your training, and the final one 2 - 3 weeks before you finish your post. The SHO log book contains all the requiste paperwork for these appraisals, and it also contains the VTR2 forms which has to be signed off by the consultant. The VTR2 form should then be countersigned by the Director of Postgraduate GP training and returned to you for your keeping. This is an important document, which will be required by PMETB from all 4 hospital posts before your training is accredited.

The SHO logbook can be downloaded using the same link as above. Please print it out and use the necessary papers for your appraisals.
